River North Bradley, a 120-bed skilled and intermediate care facility in Bradley, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the third quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
River North Bradley was cited with an A violation in August and fined $25,000.
