City of Kankakee issued the following announcement on Feb. 21.

Was your vehicle damaged by hitting a major pothole? Identify if the incident occurred on a City Street (in BLUE) or a State Route (in Red)? and follow the steps below.

STEP 3: If the damage occurred on a City Street (in BLUE) follow the procedure below:

Fill out the Motor Vehicle Damage Claim Form which is available online. (Motor Vehicle Claim.pdf).

This form can be mailed to or hand delivered to:

City Clerk / City of Kankakee

Attn: Claims

This Motor Vehicle Damage Claim Form will be reviewed and submitted to the City of Kankakee’s Insurance Company for claim eligibility and discretion.

STEP 3: If the damage occurred on a State Route (in Red) follow the procedure below:
